An Argument for Cerebral Oximetry


 Initial Blood Pressure and Cerebral Oximetry

A number of orthopedic surgeons in our practice prefer the sitting position and relatively low blood pressures for shoulder surgery.  Other than insuring the neck is in neutral position and keeping the blood pressure within certain parameters, cerebral profusion is impossible to quantitate with current monitoring.  -- SS

67 year old male with well controlled hypertension presented for SAD.  The surgeon prefers the sitting position.  Cerebral oximetry alerted me to a drop in perfusion before blood pressure cycled.  After correcting the blood pressure with 200 ug of neosynephrine, cerebral oximetry recovered before the blood pressure cuff recycled.  This monitor is useful when working with slow orthopods in cases involving old people in the sitting position.
